MAKE A RESERVATION

  • WHICH CLASS ARE YOU MAKING RESERVATIONS FOR? *
  • If your class is not listed, choose OTHER
  • CHOOSE CLASS DATE - IF IT IS A MULTIPLE DAY CLASS, SELECT FIRST DAY OF CLASS OR WORKSHOP.
  • Let us know how many students you will send. We DO NOT need their names at this time but you may add them below in the comments section if needed.
  • If you are attending on your own and not being sent by an agency. Just list your name or type "sending myself". Otherwise, list agency name.
  • CONTACT PERSON *
  • List the name of the contact person. Usually the person making the reservation or the person that should receive class info and invoices.
  • The address where we should send class info or invoices. If you want info sent one place and invoices sent another, make notes at the bottom of the form.
  • If you have an extension number, list it in the notes below.
  • We sometimes FAX confirmation info if e-mail is not working. Please list your FAX number just in case.
  • Usually your agency e-mail address. If your server bounces e-mails often, you can list an optional (personal) e-mail address below.
  • Optional field. List an alternate e-mail if you have a very secure fire wall that may not accept outside e-mails.
  • Optional field. You can list student names, ID numbers or purchase orders here. Anything you think we should know. Also, if we are offering multiple classes in your area and you want to say something like "add 2 students to the June 2nd class and 3 students to the June 3rd class, that is fine. It saves you filling out this lovely form numerous times.
  • There is no obligation, we will just send you information. It's a great way to get quality, affordable training to your region!
